    • What problems you are having
    • The history of these problems
    • Related problems
    • Family information that relates to your problems
    • How youv’e tried to manage these problems in the past

3.  We will ask you to sign release of information forms for other providers.

4.  The person interviewing you will discuss your situation with the Walk-in Supervisor.

5.  You will meet with the evaluation team (the interviewer and supervisor) to review the recommended treatment options.

The entire process may take as long as 2-3 hours.

How to Make this Process Work Best for YOU

  •   Be honest when answering questions.  Understand that we must ask personal questions to try to be the best help to you.
  •   Ask questions if you don’t understand something.
  •   Do your best to provide accurate dates and addresses for Releases
  •   Be patient sometimes thorough evalutaions require that we ask questions that seem unnecessary and that the team carefully review the information you have provided.

How will my Treatment be Paid for?

  •   We accept MassHealth and Medicare, as well as several commercial insurances
  •   We offer a very reasonable sliding scale of payment for people without insurance.  A self-pay fee will be set at Registration.
  •   If you receive DMH services we can assist you in applying for MassHealth and/or Medicare

*No one is turned away because of an inability to pay*
